Personal Preference Care Plan

What is a Personal Preference Care Plan (PPCP) and why do I want one?

Imagine you are in the position of not being able to speak for yourself. You were in an accident or have had an illness that leaves you requiring home help to get you ready for your day or maybe to get through your day. Imagine not being able to share your food preferences, how you like to dress, what you like to wear or even what you like to eat! As an experienced Care Partner I can share how valuable a document like this can be to us. If you are unable to communicate, when we come to help the only information we have to go by is what your family member or loved one gave us. It can be so frustrating for both parties. We just want to help and you just want to be helped.

You deserve to be helped in a way that keeps you comfortable and within your personal preferences.

Is there any reason why you wouldn’t want to give yourself peace of mind?

I’ve put together a list of common questions a care partner may have about how to best help you with your personal needs. You will find simple questions with some answers provided that you may choose from by circling or checking off as well as space provided to write in your specifics. It’s quick and easy.

Print the document and fill in the blanks, place it with your other important papers and you're done! It’s that simple!

Making memories while keeping memories…….

Keeping on the topic of preparations, I list below some other suggested ways to prepare a keepsake of memories.

The “Tell Me Your Life Story” books are a great item to add to your photo collection. Picture books are a great way to provide stimulation and can ignite a conversation with your loved one.

As a volunteer, I am limited to how many people I can help in the local community, with this website I’m able to provide a variety of helpful information to the masses, and as an Amazon affiliate, I earn from qualifying purchases which allows me to continue to volunteer.
