You will find simple questions with some answers provided that you may choose from by circling or checking off, as well as space provided to write in your specifics. Print the document, fill in the blanks, place it with your other important papers, and you're done! It’s that simple!
What is a Personal Preference Care Plan (PPCP) & why do I want one?
Imagine being unable to speak for yourself. An accident or illness has left you relying on someone else to help you get dressed, prepare your meals, and support you throughout your day.
Now imagine not being able to tell anyone your preferences—what you enjoy eating, how you like your meals prepared, the clothes you feel most comfortable wearing, or the little routines that make you feel like yourself. Without a way to communicate these personal choices, even the simplest parts of daily life can become frustrating and isolating.
You deserve to be helped within your personal preferences.
As an experienced Care Partner, I can share how valuable a document like this can be to us. If you are unable to communicate when we come to help, the only information we have to go by is what your family member or loved one gave us, which is frustrating for both parties.
